Clapham Family Practice aim to provide high quality, safe, professional primary care services for all our patients, and, to continue to do so in the current & future climate of healthcare.
The practice provides essential services for people who have conditions from which they are expected to recover, chronic disease management & general management of terminally ill patients. Focusing on providing proactive management of long term conditions, prevention of disease by promoting health & wellbeing, offering care, support & advice to our patients.
We aim to work in partnership with our patients, their families & carers, by working with other agencies towards a positive experience & understanding, which, involves them in decision making about their treatment & care. To deliver a whole patient centered holistic care & support for families & carers.
We aim to treat patients as individuals with respect, understanding, while supporting their needs & enabling patients to express their needs to maintain the best level of independence, choice & control.
We aim to act with integrity & confidentiality to ensure robust information governance systems.
Clapham Family Practice has a Patient Participation Group who work in conjunction with the practice, providing feedback on the services we offer & suggestions to improve the practice, as we continually strive to improve what services we are able to offer our patients.
As a Primary Health Care General Practice we ensure all staff have the competency & compassion to deliver the required services to care for our patients, ensuring the team have the right skills & training to carry out their duties competently.
The practice takes pride in offering all our staff support & training to enable them to do their jobs & protect them against abuse. The practice has a ZERO Tolerance of all forms of abuse and treat all patients and staff with dignity & respect.
We aim to provide our patients & staff with a friendly, positive & safe environment. The practice works with IRIS (Identification & Referral to Improve Safety) against Domestic Violence, which identifies the practice as Domestic Aware Abuse Practice.
Clapham Family Practice have qualified for a Gold Pride in Practice award from the LGBT foundation, the practice is proud to have been the first practice in Lambeth to obtain this qualification.
